FINANCIAL INCENTIVES FOR HEALTHY CHOICES
HEALTHpact plans have two levels of employee cost-sharing, Advantage and Basic. The Advantage Level has significantly lower deductibles, copays and co-insurance than the Basic Level. Employees commit to the 5 HEALTHpact Principles to get Advantage Level cost-sharing.

CAN SOME EMPLOYEES BE IN BASIC AND SOME IN ADVANTAGE?
Yes. An employer group premium is the same whether all employees are in Basic, all are in Advantage, or if there is a mix.

QUALIFYING FOR ADVANTAGE IN YEAR 1
To qualify for the Advantage Level for the 1st year, an employee must complete the enrollment paperwork correctly. This includes:
  1. selecting a primary care doctor
  2. making a pledge
  3. completing a Health Risk Assessment
No matter how sick you are, you are eligible for the Advantage Level in the first year of enrollment simply by completing the forms correctly.

QUALIFYING FOR ADVANTAGE IN YEAR 2
To continue to quality for the Advantage Level in your 2nd year, you must have:
  1. visited your doctor by the 6th month of the 1st year
  2. submitted a participation checklist by the 8th month of the 1st year
  3. participated in disease or case management if medically applicable
